Our Adult Family Home offers a comfortable and supportive residential setting designed to ensure every resident receives individualized attention, respect, and dignity.
We focus on creating a family-like environment where residents feel safe, valued, and supported. With a low resident-to-staff ratio, our caregivers are able to build meaningful relationships and provide tailored care that meets each resident’s unique physical, emotional, and social needs.

LSI Care Services offers residential and community-based support for adults aged 18 and older who require assistance with Activities of Daily Living (ADLs), as well as individuals with intellectual and developmental disabilities, mental health needs, and other complex care requirements. All services are designed to promote resident autonomy, health, and quality of life, in keeping with Wisconsin DHS and MCO guidelines.
Eligibility varies, but generally includes adults (18+) with developmental disabilities, serious mental illness, or behavioral health challenges. Funding must be secured through Medicaid, a managed care organization, or a referring agency.
Referrals can be submitted through our website, by calling our admissions team directly, or via email. Once a referral is received, our team will review your information and reach out within 1-3 business days.
Admission determinations are made in collaboration with residents, their families, healthcare providers, and case managers, to ensure that care needs can be safely and appropriately met.
We strive to incorporate the individual’s preferences and goals at every stage in our placement process to ensure a good fit.
We accept Medicaid, Managed Care Organization (MCO), and some private pay arrangements. For additional clarification, please reach out directly and our team can help with navigating funding questions.
Upon admission, residents receive a personalized care plan that incorporates input from the individual, family/guardian, and clinical team.
Our visitation policies are specifically designed to encourage family involvement and support frequent communication and engagement.
LSI Care Services employs a number of tools to ensure individual goals are met and services remain effective, including tracking data, conducting periodic assessments, ensuring regular touchpoints with care team, and sharing update reports with relevant stakeholders.
